In consideration of Broken Arrow Public Schools granting the student permission to participate in the camp, I hereby assume all risks of his/her personal injury that may result from camp activity. As parent/guardian, I do hereby release Broken Arrow Public Schools from all liabilities, including claims and suits at law or inequity, for injury which may result from the student taking part in camp. I hereby certify that my child is physically fit to participate in an active physical program and I hereby certify that Broken Arrow Public Schools shall not be held responsible for any consequences resulting from such injuries. I hereby certify that BAPS has full and unconditional authority to render first aid and refer to diagnosis any injury occurring during camp activity. BAPS shall not be held responsible for any consequence resulting from such injuries.
